IMPORTANT NOTICE TO ALL PLAYERS
Playing Membership Application Form & Subscriptions for 2011/2012
There are 3 ways to pay your subs. this season :
1. A one off payment of £100.00 to be paid by August 26th 2011
2. £30.00 per quarter to be paid end of AUGUST, NOVEMBER, FEBRUARY 2012 & MAY 2012
3. Standing Order payment - £10.00 per month for 12 months.
Having committed yourself to one of the above methods of payment :
1. You have the right to play for Mistley RUFC
2. Paid Insurance
3. A Club Tie
4. A 100 Club number. This will given to you at the start of the new draw which commences February 2012

AN APPLICATION FORM MUST BE COMPLETED - THIS CAN BE DOWNLOADED FROM THIS WEBSITE - Click 'Club Info' on Homepage, click 'Documents' on left hand side & select Application Form & click 'download'.
THIS APPLICATION FORM MUST BE RETURNED, COMPLETED, TO DAVE WOODS [Hon.Treasurer], by AUGUST 12th.
