Before returning to rugby league activity, all players must complete the Player Declaration Form. Please note, if you completed a 2020 form there is no need to complete a new form.

All players must opt IN by completing this form to acknowledge the risks involved and agreeing to the code of conduct. Parents & guardians should complete and sign the form for junior players.

Failure to complete the declaration will mean players are NOT allowed to take part in any activity.
