JTA has your approval to use your photo & name in any promotional material in relation to any event You understand that photographs/video footage may be taken during an event. These images may be used in coaching resources, on the JTA web site & Facebook page or for general JTA publicity & media purposes.
If you do not wish your/your child’s image or name to be used in such a way please contact: secretary@jerseytouch.org
Your data is kept for JTA administration & communication purposes.
The JTA will use the information on this form for tournament/event & medical administration and for the purpose of contacting emergency contacts in the event of a medical issue or safeguarding concern arising, the JTA may disclose certain information to relevant personnel as required.
By submitting the registration form for membership of Jersey Touch you agree to be bound by the Jersey Touch Association Constitution and any Rules and Policies made pursuant thereto
You acknowledge that you take part in JTA activities at your own risk and understand the nature of the activity and the risks involved including possible injury to yourself and/or damage to your property.
It is your responsibility to ensure that you are insured, if necessary, and sufficiently fit to take part in JTA activities and if you are in any doubt about this you will seek advice from your own General Practitioner before taking part.
You hereby release the JTA, its officers and volunteers from any liability for personal injury, death, loss, property, claim or damage suffered by you or any other person during
or as a result of your participation in JTA activities
Insurance
JTA has public liability insurance which covers activities generally but not the participants whilst actually playing. If you require personal accident or medical insurance then you must arrange this yourself.
